An exciting opportunity has arisen to work as an Assistant Practitioner within the Flying Start Health Visiting team in Pembrokeshire.
As an Assistant Practitioner, you will work as part of a skill mix team with families in their homes or community settings, delegated by the Health visitor, to improve the overall wellbeing and health of children and families.
You will deliver evidence-based strategies and key messages from the Healthy Child Wales Programme to support the best possible outcomes for children and families.

Main duties of the job
The Successful Applicant Will:
* Undertake home visits to children/families, implement a planned programme of care, and report progress/changes to the delegating Registered Practitioner.
* Prepare information packs, make appointments, and participate in child health clinics under supervision.
* Provide advice to families regarding childcare, nutrition, sleep problems, and behaviour.
* Follow-up with parents/children who default on appointments.
* Reinforce key public health messages to children and parents/carers provided by the relevant practitioner.
* Advise on safety issues in the home and community.
* Facilitate group work.

Person Specification
Essential Qualifications and Knowledge:
* Good standard of education, GCSE English and Maths (Grade A-C) or Level II equivalent.
* NVQ/QCF Level 4 or equivalent knowledge, or NVQ/QCF Level 3 plus additional training to Diploma level or equivalent.
Desirable: Community experience
Experience: Experience in health visiting, school nursing, children's social care, or education.
Language Skills: Welsh Speaker (Level 1) is desirable.
Post subject to Disclosure and Barring Service check. Applications from those requiring Skilled Worker sponsorship are welcome.
