Key Responsibilities
* Administer skilled nursing care under RN supervision.
* Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
* Utilize clinical expertise and nursing education to make informed, independent decisions that support patient care and promote positive outcomes.
* Complete visit reports promptly and accurately within the electronic medical record system to support high‑quality care and regulatory compliance, including all required documentation.
* Confirm daily visit schedules to ensure seamless coordination with other staff.
* Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
* Understand and adhere to the agency’s process for managing and addressing customer complaints.
* Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
* Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.
Qualifications
* Must have a current LPN or LVN license (state‑specific).
* Graduate of an accredited nursing program.
* Valid BLS certification.
* Minimum one year of nursing experience (home health or medical/surgical preferred).
* Excellent communication, organizational, and time management skills.
* Must have a valid driver’s license and access to a reliable and insured vehicle.
* Ability to visit homes that may not be wheelchair accessible.
* Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
* Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
