Nurse Plus are delighted to be working with a long established client in the Calderdale area.

We are seeking an experienced Clinical Nurse Lead to work in a small bedded learning disabilities home in central Halifax, West Yorskhire.

This is a permanent position directly with our client and not an agency position.

The service supports adults with learning disabilities and complex physical and mental health needs, delivering person-centred, high-quality clinical care in line with regulatory and best practice standards.

The Clinical Lead will provide strong clinical leadership, governance, and oversight across the service, ensuring safe, effective, and compliant care delivery. The role combines direct clinical responsibility with staff development, multidisciplinary collaboration, and strategic input at service and organisational level.

The role

Clinical Governance and Quality Assurance

Ensure care systems are effectively monitored and maintained to a high standard.
Oversee the monthly evaluation of care plans and risk assessments.
Ensure medication management systems are robust, compliant, and audited, including:

Ordering, booking in, and monitoring medication within monthly cycles.
Conducting medication and antibiotic audits.
Leading the monthly medication changeover (typically the last Sunday of each month, 20:30–23:00).

Ensure infection prevention and control standards are upheld throughout the service, both clinically and environmentally.
Ensure safeguarding standards are upheld, concerns are appropriately actioned, and statutory reporting requirements are met.
Ensure compliance with Local Authority, Integrated Care Board (ICB), and regulatory standards, completing action plans as required.
Clinical Leadership and Specialist Care

Provide clinical leadership in the management of complex health needs, including but not limited to:

End of life care
Catheter and stoma care
Pressure care
Diabetes, epilepsy, and cerebral palsy
Mental health conditions including bipolar disorder and schizoaffective disorder

Hold and chair MDT meetings for individuals experiencing clinical deterioration, working closely with families and professionals.
Liaise effectively with external professionals, including GPs, District Nurses, Learning Disability Teams, SALT, and Occupational Therapists.
Staff Support, Development, and Performance

Support and develop the nursing team through:

Supervision, probationary reviews, and performance management
Preceptorship and ongoing professional development
Medication competencies and clinical upskilling
Support with NMC revalidation
Identifying and sourcing external training opportunities

Support medication competency development for:

Support Workers
Supported Living Supervisors (across associated services)

Support staff performance management processes, including supervisions and annual appraisals.
Attend and contribute to nurses’ and support worker meetings to promote engagement, development, and professional standards.
Management and Operational Support

Support management oversight with:

Staff rotas
Audits
Monthly changeover of monies
Development of monthly staff timesheets for payroll

As a member of the management team, attend meetings at Head Office, including:

Quality and compliance meetings
Financial discussions
Policy reviews
Monthly service performance meetings

Provide clinical advice and support to other services within the organisation, including supported living services with an ageing demographic, contributing to service-wide upskilling and improved care outcomes.

Requirements and job detail

The job is Monday to Friday each week - 36.75 hour per week

Our client will be holding interview in coming weeks, with a view to starting early March.

You must be able to work work a few clinical shifts (days/nights) to assist the team covering leave if needed.

Valid NMC required, and have 2 years post registration experience.

Knowledge and experience of a wide range of approaches to nursing, communicating, and managing patient care.

Have up to date knowledge of current safeguarding legislation, procedures, and practices.

Evidence of managing teams/individuals.

Ability to demonstrate clinical expertise.

Act as a professional role model.

Exceptional communication skills (written and verbal) with the ability to demonstrate fluency, clarity, and effectiveness at all levels

Ability to demonstrate a safe and supportive environment for staff

Ability to prioritise workload and delegate effectively.

IT Literacy

Ability to undertake any travel in connection with the post.

Ability to work flexibly to respond to the needs of the business.
