Assistant Practitioner, Discharge to Assess

Overview

The post holder will independently manage a designated caseload of individuals. This will include assessment of Activities of Daily Living within the home environment, developing patient-centred, goal-focused care plans, and rehabilitation aimed at maximising independence to enable individuals to remain in their own home. The postholder is expected to work independently and flexibly to ensure service users’ care needs are met during the period of assessment and rehabilitation. Supervision will be available from the Care at Home Manager and registered Therapists (OT or Physiotherapy). Support is available from relevant integrated team professionals. If needs require a registered professional, the case will be passed to that professional or managed jointly. The service provided should be consistent with local and national standards.

Full driving licence and access to own vehicle are required. Registration with the Scottish Social Services Council (SSSC) is required within 6 months of commencing in post. The working hours are 31.1 hours per week, on a 4 days on, 4 days off rotation.

* Ensure care needs are met during assessment and rehabilitation, with appropriate supervision and multidisciplinary support.
* Coordinate with the Care at Home Manager and registered Therapists (OT or Physiotherapy) as needed.
* Work flexibly to meet service needs and ensure consistency with local and national standards.


Qualifications and Requirements

* Full driving licence and access to own vehicle.
* Registration with the Scottish Social Services Council (SSSC) within 6 months of commencing.
* 31.1 hours per week in a 4 days on, 4 days off rotation.
