PCN Manager and Digital & Transformation Lead

We are looking for an experienced healthcare manager to support the effective delivery and strategictransformation of services across the PCN. This is a flexible and collaborative role, working in closepartnership with the Clinical Directors, practice leads and wider system stakeholders. The individual willplay a key role in driving innovation, service integration, and continuous improvement within GeneralPractice, in accordance with the requirements of the Network Contract DES and aligned with our localstrategic priorities.

About us

Icknield Primary Care Network comprises five GP Practices covering a population of approximately 58,500 patients across Letchworth, Baldock and Ashwell in North Hertfordshire.


Job responsibilities

Job Title: PCN Manager and Digital & Transformation Lead

Job Type: Fixed Term to 31 March 2026 in line with Network Contract DirectedEnhanced Service (DES) Specification.

Hours of Work: Up to 37.5 hours per week

Salary: Depends on experience

Location: To be agreed but the postholder will primarily be home-based with travel toany Icknield Primary Care Network GP Practice premises.

Reports to: PCN Clinical Directors

Accountable to: PCN Clinical Directors

Main Duties and Responsibilities

Operational Management:

- Leading the coordination and delivery of existing and new PCN projects, including those linkedto the DES, IIF and Enhanced Access Service

- Supporting workforce planning and management of ARRS roles

- Ensuring the smooth day-to-day operation of the PCN including - governance, compliance, andcontract management

- Maintaining a good knowledge of key policies from relevant government departments andagencies, and support PCN staff to understand and interpret these locally

- Overseeing budgets and funding streams related to PCN activity

- Preparing financial reports in line with PCN objectives

Stakeholder Engagement and Communication:

- Building and maintaining effective relationships with local GP practices, Hertfordshire andWest Essex ICB and other stakeholders

- Representing the network at external meetings

Data and Performance Monitoring:

- Work in partnership with the ICS to identify relevant data sets and to support standardisationin the use of measures for understanding change and improvement in general practice.

- Presenting and communicating data to practices and PCN staff at all levels to increaseunderstanding of operational challenges

- Interpreting data and highlighting variation to support open discussion about where variationis and is not warranted

Digital and Transformation:

- Working with Hertfordshire and West Essex ICS to support the development of overallprimary care transformation strategies, including digital transformation strategies and theirdelivery

- Supporting the PCN and member practices to adapt and apply specific policies and initiativesto local needs and to prioritise initiatives based on local requirements

- Identifying best practice and learning from other areas that could be applicable to the localPCN and communicate this to staff.Developing and managing an overall transformation plan for the PCN, including identifyinginterdependencies, managing risks, considering the potential impacts on the widerorganisation and determining resource requirements.

- Supporting practices and staff to effectively use existing and new technology systems andproducts to enable improvement

Administrative Oversight:

- Providing administrative support to Clinical Directors and ensure the smooth running of PCNoperations

- Leading relevant working groups, meetings and other forums across the PCN to agreechanges, share learning, discuss challenges and resolve issues

Health and Safety:

The post-holder will assist in promoting and maintaining their own and others health, safety andsecurity as defined in the practice Health & Safety Policy, to include:

- Using personal security systems within the workplace according to practice guidelines.

- Identifying the risks involved in work activities and undertaking such activities in a way thatmanages those risks.

- Making effective use of training to update knowledge and skills.

- Adhere to all aspects of the Health & Safety at Work Act and Control of Substances Hazardousto Health (COSHH) regulations, ensuring good hygiene, cleanliness and a safe environment inthe dispensary.

Training will include mandatory training and role-specific training, as appropriate; all of which must becompleted in a timely manner.

Qualifications

* Educated to degree level or equivalent experience in a relevant field
* Management or leadership qualification (e.g., ILM, NVQ, PRINCE2)
