As a Health Visitor youll join a team who are focused on clinical excellence! Your days will be rewarding and variedand include delivering a high quality, proactive service for children and the 0-5 Healthy Child Programme. The aim of the service is to reduce health inequalities, safeguard children and improve outcomes.

Main duties of the job
As a Health Visitor within HCRG Care Group, you'll join a team who are focused on making an impact on the development and wellbeing of children in our local community.
You will:
1. Lead and deliver the Healthy Child Programme within a designated population area
2. assess health needs and implementing interventions with a focus on personalised or targeted responses, utilizing team skills effectively.
3. Take accountability for children's outcomes collaborating with partners to address public health needs
4. Promote health and well-being through the Healthy Child Programme and referrals to partner agencies as needed
5. Contribute to multi-agency early support assessments for children and families, taking on a lead professional role when necessary.
6. Manage a caseload autonomously ensuring effective assessment, intervention, and management.
7. Model excellence in Specialist Community Public Health Nursing practice, providing supervision to learners and colleagues.
8. Provide management and leadership to community nursery nurses and other team members, including caseload allocation and monitoring.
