We are seeking a Field Care Coordinator to join our team at a busy office providing domiciliary care. The role is fulltime and will be based both in the office and in the field. The ideal candidate will have excellent IT systems and communication skills, have an even temperament, be patient and have an exceptionally high attention to detail.

Shifts available: Full time. Permanent Monday to Friday – flexibility in hours – (on-call and weekend availability as required)
Who can apply:

About the Role:

Duties and responsibilities will include:
* Ensuring rostering systems are implemented through the electronic care monitoring system and all carers have their calls assigned.
* Ensuring all service users' calls are assigned to planned calls.
* Understanding and meeting our client needs
* Supporting and undertaking regular supervision with your care team
* Supporting your care team with calls as and when required
* You will be responsible for implementing field-based quality control systems and providing support to your care team.
* Arranging initial visits to, and the assessment of, prospective Service Users to decide whether Sama Care Ltd can meet their personal care needs
* Conduct Risk Assessments as part of the Individual Needs Assessment and to update these as required as part of the Service User's risk management;
* Participate in the on-call rota
* Identify any training needs of Care Workers and report accordingly.
* Monitor, supervise and mentor all Care Workers in the field
* Encourage and identify opportunities for Care Workers' continuing professional development.
* Ensure the accuracy and completeness of all documentation held in Service Users' homes.
* Ensure that all written reports and administration is up to date
* Liaise closely with the office and report back regularly on activity in the field.

* Completing QCF Level 3 in Health and Social Care or equivalent qualification and upwards.
