Job Title: Community Rehabilitation Assistant

We are seeking a Community Rehabilitation Assistant to join the Christchurch & Bournemouth Intermediate Care Team. This role offers the opportunity to be part of a service that is continually evolving to meet community needs.
As a key team member, you will support the delivery of effective rehabilitation for individuals in their homes and care settings. You will collaborate with physiotherapists, occupational therapists, nurses, and other professionals to:
* Prevent avoidable hospital admissions
* Support recovery after illness or injury
* Promote independence and quality of life
You will also contribute to the Community Hospital at Home service, with opportunities to develop clinical skills such as taking bloods and ECGs, as part of a multidisciplinary team. Your responsibilities include providing rehabilitation support to help patients regain strength, confidence, and independence following illness or hospital discharge. You will work under supervision, supporting individualised treatment plans, managing a varied caseload across conditions like orthopaedic, neurological, respiratory, and frailty, and maintaining accurate records.
The service operates from 08:00 to 20:00, seven days a week. Flexibility may be considered to support work-life balance. A valid UK driving license and access to a car are essential for travel to patients' homes. Employment requires an Enhanced DBS check, which the Trust covers.
