Learning Disability Hospital Liaison Nurse

We are looking for an experienced Registered Learning Disability Nurse to join the Learning Disability Hospital Liaison Service across Hampshire Hospitals NHS Foundation Trust. This role supports patients at Basingstoke and North Hampshire Hospital, Royal Hampshire County Hospital (Winchester), and Andover War Memorial Hospital. The base is flexible and may be agreed between Basingstoke and Winchester. The post involves regular cross‑site work and a shuttle bus service is available between the sites.
Responsibilities
* Provide liaison support to adults with learning disabilities in acute hospital settings.
* Support and advise hospital staff to deliver high‑quality, person‑centred care.
* Identify and raise concerns appropriately, including safeguarding and incident reporting.
* Assist with smooth service delivery through supportive leadership and liaison input.
* Lead and coordinate complex patient‑centred meetings within an acute hospital environment while managing a caseload.
* Provide training to general hospital staff on learning disability topics.
* Recognise healthcare inequalities faced by people with learning disabilities and make reasonable adjustments.
* Use the Mental Capacity Act and related legal frameworks to support patients.
* Work collaboratively with multi‑professional teams and voluntary agencies.
* Build and promote effective relationships with agencies, service users and relatives.
* Use a range of communication methods and tools to support understanding.
* Plan, prioritise and document workload effectively.
* Contribute to quality improvement initiatives in the hospital setting.
Qualifications & Experience
* Registered Nurse in Learning Disability (RNLD).
* Evidence of ongoing professional development.
* Full UK driving licence.
* Proficiency in Makaton or other communication techniques.
* Experience supporting adults with learning disabilities in healthcare environments.
* Experience providing supportive leadership in acute hospital settings.
* Experience in safeguarding, incident reporting and risk management.
* Excellent computer skills, including Microsoft Office.
* Strong motivation to engage in quality improvement initiatives.
* Experience working flexibly to meet service users’ needs.
