About the role
As a Social Work Assistant within the CHC team, you’ll be a valued member of the multidisciplinary team (MDT), contributing to CHC eligibility recommendations for individuals referred for, or already receiving, care and support services. You’ll produce high-quality documentation to support decision-making and inform future assessments. Typically, you’ll be involved in 4–5 Decision Support Tool (DST) meetings per week.
You’ll work closely with professionals and partner organisations to provide holistic, person-centred support. You’ll champion the principles of personalisation, enabling individuals to retain as much control over their lives as possible.
Your responsibilities will also include screening CHC checklists, processing joint funding requests, and completing associated paperwork. You may also deliver talks and training to wider Adult Social Care locality teams. You’ll be supported by approachable managers and experienced colleagues – you’ll never be on your own.
Any-Desk:
As an ‘any-desk’ worker, you’ll need to be connected to our network to access digital folders and resources, but this can either be from home or an office location when required. You will be required to work from the office a couple of times a week, depending on your role and team requirements.

You’ll need to be emotionally resilient, able to stay focused and positive even in challenging situations. The ability to travel is essential for this role.
