Main duties of the job
You will be responsible for delivering both unscheduled care and planned appointments, managing patients across face-to-face consultations, telephone triage, and home visits. The role involves managing presentations and making clinical decisions independently.
You will work collaboratively with the wider clinical team to ensure care is delivered safely and efficiently, while also contributing to a positive patient experience and the smooth day-to-day running of services.

Job Description ANP
Job Title: Advanced Nurse Practitioner Reporting to: GP/ANP Partners / Operations Manager / Business Manager
Key Responsibilities
1. Deliver high-quality, patient-centred care.
2. Conduct face-to-face and telephone consultations, and carry out home visits.
3. Assess, diagnose, treat, and discharge patients or refer as appropriate.
4. Prescribe safely and effectively.
5. Work within practice triage systems to manage same-day demand.
6. Provide clinical advice and guidance to patients and the wider team.
7. Maintain accurate, contemporaneous clinical records.
8. Support health promotion and self-care where appropriate.
9. Work collaboratively within the multidisciplinary team.
10. Contribute to clinical governance, audit, and quality improvement.
11. Review and action patient correspondence.
12. Responsible for requesting and acting on investigations requested including blood results.
Person Specification
Essential
13. Registered Nurse with valid NMC registration
14. Advanced Clinical Practice qualification at Masters level (or equivalent).
15. Experience managing minor illness and acute presentations in primary or urgent care.
16. Independent Prescriber (or working towards).
17. Strong clinical assessment and decision-making skills.
18. Ability to work autonomously in a fast-paced environment.
19. Excellent communication and patient care skills.
20. Full UK drivers licence .
