Overview
Band 5 Community Nurse - Extended Primary Care Teams, Newham. This is an opportunity for experienced and newly qualified Registered Nurses (Adult) to join our Extended Primary Care Teams in Newham, East London as Band 5 Community Nurse.
The service is based in East Ham Care Centre and operates 365/366 days a year including weekends and Bank holidays from 8am to 8pm. East London Foundation Trust values its staff and provides regular supervision, appraisal, and opportunities for training and continuing professional development.
Responsibilities
* The post holder should be able to work autonomously to provide nursing care to patients in their own home.
* The post holder will be responsible for the planning, implementation, and evaluation of individualized nursing care in a range of settings.
* The post holder will provide health education to enable patients to self-manage and achieve optimum independence following deterioration or a period of ill health.
* The post holder will support the senior nursing team in the maintenance of efficient and effective delivery of services.
* Work closely with multidisciplinary teams including GPs, social workers, therapists, and specialists.
* Coordinate care to ensure continuity and prevent hospital admissions.
* Communicate effectively with patients, families, and healthcare professionals.
What We Are Looking For
* Experienced qualified nurses (Registered Nurses Adult) who want to develop skills and knowledge in community health practice.
* Newly qualified nurses seeking their first post as a qualified nurse.
* Nurses who offer person-centered nursing care and have excellent verbal and non‑verbal communication skills.
* Nurses who can work as part of a multidisciplinary team to enhance the quality of care provided to patients.
* Nurses able to work shifts over 7 days a week, including Bank Holidays.
Qualifications
Essential
* Registered General Nurse (NMC) and current registration in Adult Nursing.
* Demonstrate evidence-based practice and up-to-date clinical skills.
Desirable
* Evidence of post-basic professional development.
* Relevant teaching/mentorship qualification or commitment to undertake.
Experience and Knowledge
* Evidence of and commitment to multi-professional working.
* Literate in IT/Computer Skills.
Other
* Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings.
* Desirable: Current driving license.
