Overview
We are looking to recruit someone to join our Integrated Neighbourhood Team (INT) at Hedena Health as a Care Coordinator, playing a key role in supporting patients with complex needs to improve access to healthcare services. This is a fantastic opportunity to be part of a pioneering NHS project focused on proactive, community-based care. The key duties is to develop and implement proactive care services to help improve and maintain the health and wellbeing of our practice population. Proactively identify and work with a cohort of patients to support their personalised care requirements.
