What the Job Involves The job involves supporting clients to achieve their individual goals and providing assistance at home to prevent unnecessary hospital admissions and facilitate discharges. You will work closely with health professionals and follow a service delivery plan to guide your daily duties, using a mobile device to receive task allocations. Flexible working hours, including evenings and weekends, are part of the role, and a rota system will determine shifts. This position is permanent for 37 hours per week, and adherence to safeguarding and child protection standards is paramount.
