Are you an experienced Advanced Clinical Practitioner (ACP)? If so, then we want to hear from you!

We’re looking for an ambitious, enthusiastic, motivated and experienced ACP to join our team, someone who will drive clinical quality and provide outstanding clinical leadership. The applicant will be required to work as an autonomous practitioner identifying, assessing, diagnosing and treating older adults with frailty needs while taking the next steps in developing community services. Exciting changes are underway within our Community Assessment Unit focussing on the clinical pathways and clinical care provided to our older frail patients, expanding on our current frailty assessment offer to a 7 day service aiming to build upon this to a 24 hour/7 day model. If you have a strong interest in reablement, working with patients' strengths and promoting their autonomy by enabling safe discharge to place of residence to maintain their independence in the community, this could be for you.

NHS experience is essential.

This full time role (37.5 hours) will support the service delivery across a 7 day 8am-8pm offer, with a view to moving to a 24/7 model as part of our future improvement works.

An Enhanced DBS (with Adults Barred List check) will be obtained for the successful candidate during pre-employment checks. Applicants must hold professional registration from a Nursing or Allied Health professional background.

Main duties of the job

The Community Assessment Unit (CAU) consists of 21 beds including 9 single rooms. Its primary function is to operate as an older persons assessment unit, accepting patients for step up/step down care and safe discharge into the community. The role is to assess, diagnose and treat patients independently referred from Accident and Emergency, North West Ambulance and the Community React Team and step down patients from the acute site, in providing the right care in the right place and ensure a safe and timely discharge for this cohort of patients in our six chair Frailty SDEC and to take an active role in the management of the 21 bedded ward area alongside the wider MDT.
The role will require work across all areas of the Trust, including those designated as positive, asymptomatic and negative COVID areas.
This is an exciting time for the Community Assessment Unit as we continue to evolve, working closely with our key stakeholders. The expectation is to work within a changing environment and to drive the transformation agenda.

On-Call

Detailed job description and main responsibilities

Duties and Responsibilities

Key results from the job holder

1. The post holder will be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care
2. The post holder will be responsible for providing face to face consultations for patients in the Community Assessment Unit and Frailty SDEC
3. The post holder will provide clinical leadership and support for members of the multidisciplinary team
4. The post holder will be able to take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgement to identify the potential diagnosis

Planning and Organisational Duties

5. Work within the Trust's Policies and Procedures and the parameters set down in agreed local clinical protocols and guidelines and national guidelines such as those published by NICE.
6. Organise the clinical management of individual patients, utilising clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes where required. This will include monitoring and evaluating the responses to treatment through analysis and interpretation of clinical tests and investigations and the discharge of the patient.
7. Apply appropriate advanced clinical and technological skills in response to the changing needs of care and the dynamic state of healthcare services.
8. Facilitate the development of clinical practice through research and education of the multidisciplinary team, by integrating theory with practice.
9. Evaluate care through quality assurance initiative and assessment of clinical outcomes.
10. Be an expert clinical resource for other health care practitioners.
11. Take every opportunity to promote the health of patients and initiate health education strategies as necessary.
12. Participate in risk management and risk assessment working closely with other members of the clinical team.
13. Continually develop own professional practice as indicated by the changing needs of services / patients
14. Participate in the development of the quality and audit strategy, which includes standard setting and development of audit tools.
15. Participate in developing and updating clinical protocols in care in responses to advances in clinical practice.

Communications and Key Working Relationships

16. Accept, make and receive referrals to and from other health care professionals.
17. Communicate patients' conditions, verbally and in writing, to other members of the healthcare team and contribute to decision-making processes in patient management.
18. Communicate on an on-going basis with the GP, Community Medical Lead, Senior Registrar / Consultant responsible for the patient - updating him / her on changes / progress.
19. Participate in ward rounds, perform case presentation and contribute to the decision making process in patient management.
20. Act as a resource, providing clinical leadership and facilitating professional development amongst the multidisciplinary team both within and outside the organisation.
21. Promote best practice, utilising local and regional benchmarking groups for the generation and promotion of standards and clinical guidelines.
22. Contribute to clinical governance arrangements by providing expert input and securing quality improvement, including influencing other disciplines, to help deliver quality services.

Responsibility for Finance

23. Ensuring the effective use of all financial resources in providing and delivering the service.
24. Identifying and implementing cost improvement programmes within the service

Responsibility for Human Resources

25. Involvement in staff engagement and patient feedback.

Responsibility for Health & Safety

Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.

Responsibility for Teaching

41. Significant clinical experience where a degree of autonomous working has been required.
42. Demonstrable experience and involvement in clinical audit methods and quality initiatives related to clinical team activities
43. Experience of leadership and experience of implementing significant change or service development
44. Experience of undertaking expanded roles within the speciality
45. Evidence of working on own initiative, managing time effectively and prioritising appropriately.
46. Experience of supporting students and other clinical staff within the clinical areas.
47. Experience of critically analysing published research and applying theory to practice
