Job Summary

The Links Medical Practice is a GP Practice that operates from 2 sites in the Borough of Bromley.

An excellent opportunity has arisen for a highly motivated individual to join our friendly team.

The position is permanent, part-time with a flexible shift pattern. Hours of work range between 7:45am - 6:45pm Monday to Friday.

The applicant(s) will be based at either site - Mottingham site - 198 Court Farm Road, London, SE9 4JS and Downham site - 27 Brook Lane, Bromley, BR1 4PX


Main Duties of the Job

We are looking for a proactive and well-organised Care Coordinator to manage the recall process for patients with long-term health conditions. This role involves ensuring patients are invited for their regular reviews on time and that all elements of their care are completed with the appropriate clinicians. The Care Coordinator will also support accurate and timely patient registrations as part of their responsibilities.

Previous experience in this role is desirable.


About Us

The practice is managed by 4 GP Partners and the Practice Business Manager.

Additional team members are a further 4 GPs, 3 Physicians Associates and 6 members of the nursing team. There are approximately 15 administrative and reception team members. We also actively collaborate with 2 other practices as a Primary Care Network team.

The practice values each other and the skills individuals contribute to the practice both in terms of daily work schedule and the delivering of patient care.

We are also a training practice for medical students and trainee nurses and we offer excellent support and training, the opportunity to gain new skills and knowledge in a friendly learning environment, and an exciting opportunity for someone who is interested in working in a well-established and growing practice.


Job Responsibilities

We are seeking a proactive and compassionate Care Coordinator to join our team. This role plays a key part in supporting patients with long-term health conditions by coordinating their regular reviews and ensuring all aspects of their care are delivered by the appropriate clinicians. You will also take responsibility for managing accurate and timely patient registrations.

* Manage patient recalls for long-term condition reviews, ensuring patients attend regular check-ups and receive complete care from the relevant healthcare professionals.
* Support patient registrations, ensuring records are accurate and up to date as part of the onboarding process.
* Work with patients, families, and carers to improve understanding of long-term conditions.
* Provide a point of contact for patients to help with queries, appointments, and access to high-quality information about their care options.
* Coordinate and navigate care across services, ensuring patients receive joined-up support from the right professional at the right time.
* Collaborate with GPs and other professionals within the Primary Care Network (PCN) to proactively manage a caseload, including patients with long-term conditions, and refer them appropriately within the team.

Essential Experience

* Ability to actively listen, empathise with people and provide personalised support in a non-judgemental way.

* Commitment to reducing health inequalities and proactively working to reach people from diverse communities.
* Ability to support people in a way that inspires trust and confidence, motivating others to reach their potential.
* Ability to communicate effectively, both verbally and in writing, with people, their families, carers, partner agencies and stakeholders.
* Ability to identify risk and assess / manage risk when working with individual.
* Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the care co-ordinator role e.g. when there is a mental health need requiring a qualified practitioner.
* Ability to maintain effective working relationships and to promote collaborative practice with all colleagues.
* Ability to demonstrate personal accountability, emotional resilience and work well under pressure.
* Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines.
* Ability to work flexibly and enthusiastically within a team or on own initiative.
* Knowledge of, and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
* Demonstrable commitment to professional and personal development.
* Proficient in MS Office and web -based services.


Desirable Experience

* Experience of working in Primary Care.
