Advanced Practitioner - Acute Services in Medicine and Frailty

An exciting opportunity has arisen to join our Acute services in Medicine and Frailty (Medicine for the elderly) at Cambridge University Hospitals Foundation Trust. The post involves working across these services as part of the multidisciplinary team and as a valued member of the Advanced Practice team.

You must have NMC or HCPC registration.

You must hold an MSc in Advanced Practice and currently be or have recently been in an Advanced Clinical Practitioner role with sufficient post Masters experience in an advanced practice post. Ideally you should have experience in secondary care - Acute Medicine and/ or Acute Frailty.

You must have portfolio evidence of post MSc learning, development and assessment.

Main duties of the job

You will be practising autonomously as an advanced clinical practitioner (ACP) within acute services to provide patient-centred quality care. This will encompass the skills of assessment, examination, diagnosis and treatment within an agreed scope of practice throughout acute services. You will support a new way of working that emphasises a more efficient and patient focused service and will ensure the safe referral and discharge of patients with undifferentiated and undiagnosed presentations across acute medicine and medicine for the elderly. This role will have a predominantly clinical component. Non-clinical time is included and will be given related to appraisal, clinical audit, teaching, self-development, development of the service and research. The amount of time is flexible to meet these requirements.

You must have a desire to learn with exceptional interpersonal and organisational skills, which are essential for this role. You will take pride in the clinical area and create a culture where the team feel empowered. The service aims to deliver and sustain highest standards of care, quality and dignity for all patients and the ACP will be responsible for delivering this.

Person Specification


Qualifications

* Relevant registered professional qualification
* MSc Advanced Practice with a significant portfolio of clinical practice
* Completed full training program as a trainee AP in speciality field
* Advanced Non-medical prescribing (as required within scope of practice)
* Advanced life support (as required depending on clinical area)
* Recognised teaching qualification
* ALS Instructor
* Recognised leadership qualification


Experience

* Team leadership experience, including effective staff management including HR issues, sickness, grievance and disciplinary management / investigation.
* Evidence of ability and knowledge on how to undertake/implement audit/research and feedback.
* In-depth understanding of quality issues including clinical governance.
* Experience in recruitment and retention matters, staff appraisal and performance management.
* Evidence of involvement in primary research.
* Evidence of audits undertaken and outcomes.
* Relevant experience of service transformation
* Evidence of presentations at local / national levels
* Implementation of audit/ research recommendations.
* Proficient with EPIC e-hospital system.


Knowledge

* Extensive post registration experience and demonstrable experience in relevant specialty area
* Significant experience in an Advanced Practice post (Minimum 18 months)
* Portfolio of evidence showing skills competence and achievements in practice speciality.
* Awareness of relevant national and local key performance indicators
* Awareness of relevant local and national strategies
* Awareness of implications of advanced practice
* Acute Medicine or Frailty experience in secondary care.
* Evidence of autonomous practice.
* Commitment to the on-going implementation of the Trust strategy.


Skills

* Ability to make decisions, organise and prioritise own workload.
