We are seeking to recruit an experienced and efficient Patient Flow Co-ordinator to join an exciting new development of an enhanced crisis / intensive home treatment service operating 7 days a week. This is a vital role responsible for coordinating referrals, patient information, data, and ensuring service performance timeframes are met.
You will be part of an initiative to provide children and young people in Sussex with a more integrated range of services tailored to their clinical needs. Working in partnership with the clinical team and Provider Collaborative, you will assist young people accessing acute care, ensuring they receive the right care at the right time, whether in inpatient beds or in the community, including intensive home treatment.
The Sussex CAMHS 'hospital at home' aims to offer an alternative to admission, supporting the young person's local network during acute mental health difficulties (aged 12-18). Your responsibilities will include managing data, overseeing patient information, and facilitating the patient journey through acute care via administrative and data management processes.
The Getting it Right First Time (GIRFT-2022) report emphasizes the importance of providing young people with treatment close to home while maintaining their social, family, and educational connections.
Main duties of the job
* Promote and demonstrate effective collaboration between inpatient and intensive home treatment services to deliver timely, efficient care.
* Offer young people and families a seamless acute care experience, keeping them well-informed and supported.
* Facilitate access to inpatient or alternative services without delay, overcoming barriers between services.
* Develop effective working relationships with local teams and services to support and optimize patient flow.
* Participate as a core member of CAMHS Acute, leading patient flow operations on a Monday-Sunday 9am-8pm shift pattern, including supporting colleagues during leave.

Job responsibilities
The post holder will oversee all referrals for CAMHS Acute (inpatient and crisis/intensive home treatment), ensuring timely, professional responses. Responsibilities include managing access and discharge points, handling data and documentation, and promoting integrated working with external agencies such as social care, Provider Collaborative, community CAMHS, LD/A services, and education providers to ensure smooth transitions and communication.
You will ensure that patient stays are appropriate and brief, maintaining high-quality care and patient experience, and work to prevent unnecessary hospital admissions and readmissions through effective coordination with community services.

Qualifications
* NVQ 3 in Business & Administration or equivalent practical experience
* RSA 3 or equivalent with further practical experience
* Good secondary education, GCSE Grade C or above

* Extensive experience with secretarial procedures and office/resource management

* Proven ability to produce high-standard, accurate documentation
