Patient Flow Capacity Support Role
The capacity team is responsible for patient flow, ensuring patients are placed in the right setting at the right time, and supporting actions related to the ED 4-hour wait target to uphold patient care quality.
The post holder will provide efficient, effective, and comprehensive clerical support to facilitate patient flow. Key responsibilities include:
1. Communicating effectively with all departments regarding updates on specific patients and supporting patient flow.
2. Updating and maintaining relevant spreadsheets, recording accurate information related to referrals and breaches.
3. Recording accurate data on actual and potential bed availability and communicating this to key contacts internally and externally as appropriate.
4. Updating information systems for use by internal and external teams.
5. Maintaining and updating MDT databases to ensure timely and accurate data entry related to bed utilization and other reports.
6. Escalating delays or concerns to relevant managers or escalation leads.
7. Booking ambulances in accordance with DOH guidelines, tracking discharges, and liaising with WMAS and other patient transport agencies as needed.
8. Supporting the implementation of the trust escalation policy.
9. Communicating effectively to address potential complaints and concerns.
10. Liaising with CCG and social care partners as appropriate.
11. Providing information to support weekly operational reports.
