Healthcare Community Support Worker – Community
Do you want to make a real difference to patients? Are you passionate about supporting patients to remain in their own homes? Would you like to be part of a multi‑disciplinary service working in the community and making a real difference to patients and their families?
We are actively recruiting Healthcare Community Support Workers for our Urgent Response and Hospital at Home service, with vacancies across Eastleigh, Southampton and Lymington. The service runs from 8am to 8pm with a maximum of two weekends a month.
Main duties of the job
* Deliver delegated clinical, rehabilitation and personal care in patients’ homes
* Undertake observations and support assessments
* Support people with frailty, long‑term conditions, mobility needs and end‑of‑life care
* Work as a lone worker with robust clinical support
* Accurately record care using electronic systems
What We're Looking For
* NVQ Level 3 (or equivalent experience) or strong care experience
* Experience in a care, hospital or community setting
* Ability to work independently and manage a varied workload
* Compassionate, reliable and good communication skills

Salary: £25,760 to £27,476 a year (pro‑rata for part‑time hours)
Location: Eastleigh, Southampton, Lymington – please indicate your preferred location(s) when applying. You will need a current driving licence and access to a vehicle for this role.
