Job overview

We are seeking an individual to support the next stages in the development of the newly established CAMHS transitions service. This role will be a pilot, and it will run for 12 months initially, but the hope is that this role will become permanent, so we can continue to support young people from our CAMHS inpatient settings to the next stage in their recovery.

As a team we adopt a holistic approach, not only focusing on mental health but also addressing the physical, social, educational, and motivational needs of the young people we work with. By employing the "social prescribing" model, we help young people engage with life through activities that promote community connections, sports, education, employment, housing, and more. Your role will empower young people to build confidence, improve wellbeing, and work towards their long-term health goals.

We work in both the inpatient and community settings. We work from Kent and Medway Adolescent Hospital. We work with multiple services, and we will often have daily communications with community CAMHS, Children’s Services, education providers, third-party services, adult services.

Main duties of the job

.

1. To contribute as a key member of the CAMHS Transitions Team, providing high-quality care to young people and developing, planning, and implementing individualised treatment programmes based on the specific needs of each young person.
2. To critically assess and evaluate the effectiveness of care programmes, working collaboratively with the multidisciplinary team to ensure optimal outcomes.
3. To provide expert clinical knowledge and skills across both inpatient and community settings, delivering evidence-based interventions and direct care.
4. To manage caseloads autonomously, ensuring effective time management while delivering care and support to young people in inpatients and community.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

6. To critically assess and evaluate the effectiveness of care programmes, working collaboratively with the multidisciplinary team to ensure optimal outcomes.
7. To provide expert clinical knowledge and skills across both inpatient and community settings, delivering evidence-based interventions and direct care.
8. To manage caseloads autonomously, ensuring effective time management while delivering care and support to young people in inpatients and community.
9. To monitor and assess the impact of prescribed medication, providing timely updates to the line manager and ensuring the wider team is informed of any changes, fostering effective communication across teams.
10. To engage young people in a person-centred approach to goal setting and treatment planning, ensuring that their views and those of significant others are integral to the formulation of care plans.
11. To actively contribute to the development and improvement of the transition service, supporting the implementation of service changes and contributing to the development of new initiatives within the team.
12. To participate in professional meetings, including multi agency risk planning meeting reviews, contributing to multi-disciplinary discussions to ensure high-quality care is provided.
13. To maintain accurate and up-to-date records of care programmes and communications, ensuring compliance with the electronic patient record system (Rio).
14. To demonstrate sound knowledge of and adhere to the principles of the Mental Health Act 1983 in practice.
15. To collaborate with the wider professional network, contributing to care planning and ensuring integrated support for young people.

To prioritise safeguarding in all aspects of clinical practice, attend relevant safeguarding training, address concerns with safeguarding leads, and ensure that safeguarding issues are appropriately recorded on the electronic patient record system

Essential criteria

16. Successful completion of a graduate training ( nursing, social work, occupational therapy) or equivalent substantial CAMHS clinical experience
17. To be eligible for registration as a full member of the appropriate professional body ( HCPC, NMC) where appropriate

Experience

Essential criteria

18. Experience of working with difficult, disturbed or challenging children/young people requiring skilled and complex interventions
19. Experience of working therapeutically with parents/carers/families of children and young people with complex mental health problems
20. Experience of assessment carried out autonomously in order to determine the most appropriate treatment plan from a range of options for the child/young person

Knowledge

Essential criteria

21. Knowledge of risk management and ability to monitor and assess risk.
