Senior Health and Social Care Support Worker
NHS Highland is seeking a Health and Social Care Support Worker to join the Care at Home Team in Lochaber. The post holder will work effectively supporting care as appropriate to promote optimum achievable self-care, allowing clients to remain in their own homes. There is a goal within Lochaber to expand the service to include reablement/rehabilitation to allow patients to avoid acute hospital admission or return home from hospital sooner and remain living in their own homes.

Applicants should demonstrate willingness to undertake continuing professional development, including the essential completion SVQ health and social care level 3 to be registered with the SSSC.
This role works within a rota system and includes flexible, unsociable hours and weekend working. Post holders will be expected to work throughout the Lochaber region with flexibility.
Current UK driving licence and use of own car for work purposes are essential.

Responsibilities
* Provide care and support to clients in their own homes within the Lochaber area, promoting self-care and independence.
* Work within the Care at Home Team to deliver care in line with local and national standards and NHS Highland policies.
* Participate in the planned development of reablement/rehabilitation services to help clients avoid hospital admission where possible.
* Adhere to confidentiality and information governance requirements at all times.
* Engage in continuing professional development and complete required training objectives identified through supervision.
* Work as part of a rota with flexible hours including weekends as required.
Qualifications and Requirements
* Essential SVQ Health and Social Care Level 3 (to enable registration with the SSSC).
* Current UK driving licence and access to own car for work purposes.
