North Coastal Cluster Community Nurse Team Manager

An exciting opportunity has arisen for a Community Nurse Team Manager with the North Coastal Cluster Community Nursing Team. The role covers 2 bases: Wadebridge & Tintagel covering a large rural area.
The successful applicant will lead and manage the operational services of a Community Nursing Locality Team, taking into account local health needs and the policy priority of developing services closer to home.
You will be responsible for the delivery and maintenance of high quality evidence based care, focused on meeting the needs and choices of patients, carers and families, fostering positive relationships with local commissioning boards and community and voluntary organisations.
Creating an effective, integrated multi-disciplinary approach you will be responsible for working within the locality teams, ensuring the best practice guidelines and performance standards are met while meeting service targets.
You will be an autonomous practitioner responsible for the assessment and treatment of patients with highly complex health and social care needs, and provide developmental leadership and management to all staff within the locality and with responsibility to form and inform future service development.
This is a community-based role that requires travel to locations throughout the county including areas that may not be accessible by public transport. Successful candidates need to demonstrate how they will manage these travel requirements.

* Responsible for Quality and Governance, including Health & Safety and patient safety.

Person Specification
Education/Qualifications and Relevant Experience
* First level registration adult nursing and registered with NMC
* Extensive experience of practising within the community, as a community nurse/District Nurse
* Experience of implementing change, developing and implementing policies
* Experience of and belief in benefits of integrated multi-disciplinary team working.
* Previous experience, knowledge and application of Safeguarding Policies
* Masters level professional education supplemented by specialist training and development or equivalent experience.
Skills and Aptitude
* Teaching and assessing, coaching and mentoring skills to facilitate an effective learning environment
* Liaison and negotiation skills, as well as highly developed communication skills
* Proven written and oral communication skills including ability to deal with challenging and sensitive situations
* Highly developed organisations skills
* Budget Management Skills
Knowledge and Abilities
* Ability to lead, manage and co-ordinate teams in an autonomous manner
