This is a full-time, permanent role with hybrid working, and a salary of £24,895 - £28,132 per annum.

The Role:
The Social Care Practitioner is part of the Front Door Team, the initial contact point for residents seeking social services. Responsibilities include providing advice, signposting to services, making referrals, supplying occupational therapy equipment, and completing statutory assessments under the Care Act 2014.
Your responsibilities will include:
1. Assessments and interventions with residents
2. Signposting and advising residents contacting the team
3. Managing a caseload
What we are looking for:
* Experience working with people in a care setting or similar
* Good IT skills including MS Office, Word, and Excel
* UK driving licence
