NHFT is contracted to provide integrated health care services to HMP Littlehey. We are looking to recruit an innovative Band 7 long‑term condition lead to our team. The post holder will deliver the highest standard of care to meet the diverse and complex healthcare needs of an adult male population. They will be passionate about all aspects of nursing, have exceptional clinical skills and hold an NMP qualification, demonstrating exemplary standards of care and professionalism.
Location: HMP Littlehey is a large Category C prison in the village of Perry near Huntingdon, Cambridgeshire.
Responsibilities
* Clinical Care
o Assess, monitor, and manage patients with long‑term conditions such as diabetes, asthma, COPD, cardiovascular disease, epilepsy, and hypertension.
o Deliver structured LTC clinics and annual health reviews.
o Develop, implement, and review individualised care plans in line with national guidelines.
* Medication Management
o Prescribe and administer medication in line with professional competence and prison policy, ensuring safe medicines management.
o Support continuity of medication on reception, transfer, and release.
o Support patients to self‑manage their long‑term conditions.
o Promote healthy lifestyle choices, including smoking cessation, diet, physical activity, and patient education.
* Multidisciplinary Working
o Work collaboratively with GPs, mental health services, substance misuse teams, allied health professionals and prison staff.
o Oversee and supervise the wider healthcare team to support them in managing LTC.
o Contribute to case reviews, MDT meetings, and care planning discussions.
o Act as a patient advocate while maintaining security and safety requirements.
* Documentation and Governance
o Maintain accurate, timely and confidential clinical records.
o Work in accordance with NMC Code, clinical governance frameworks and prison healthcare policies.
o Participate in audit, service development and quality improvement initiatives.

* Band 7 level qualification.
* NMP qualification.
