Job Introduction
An exciting opportunity has arisen for a Community Specialty Doctor to work across our 2-Hour Urgent Care Response Team, 20-bedded Community Virtual Ward, and two nurse-led Intermediate Care Community Rehab Hospitals in the Swale area. The role involves providing medical care and advice to patients across these services.
You will be part of a multidisciplinary team including ANPs, an ECP, Lead Pharmacist, Ward Matrons, Specialist Nurses, and AHP leads, delivering high-quality care to patients in their homes or residences. The goal is to facilitate early hospital discharge or prevent unnecessary hospital admissions.
The services include:
* Twenty-bedded Community Virtual Ward with defined pathways (frailty; respiratory and cardiac)
* 2-Hour Urgent Care Response Service to reduce unnecessary hospital admissions
* Two 21-bedded Intermediate Care inpatient rehab wards

Main Responsibilities
* Manage complex cases and provide consultation, acting as an expert in patient care.
* Deliver high standards of assessment and treatment in line with national guidelines.
* Conduct holistic assessments for older patients.
* Respond to health-related problems, including diagnosis and referral.
* Ensure prescribing practices comply with policies and guidelines.
* Involve patients in their care where appropriate.
* Participate in MDT ward rounds and meetings.
* Collaborate with the pharmacist to promote safe medication prescribing.
* Provide care in emergency situations as needed.
* Supervise and mentor student advanced nurse practitioners and non-medical prescribers.
* Provide teaching and training to staff.

The Ideal Candidate
* UK medical experience
* Primary medical qualification
* Experience in internal or geriatric medicine
* Willing to pursue Diploma in Older People’s Medicine
* Completed GP training
* Post-graduate NHS experience
* IT skills, including MS Office
* High standard of clinical care
* Self-motivated and proactive
* Excellent communication skills
* Proficient in English, spoken and written
* Must have a full UK driving licence and access to a vehicle
