The post holder will work with the wider multi-disciplinary team to manage and co-

ordinate the provision of diabetes nursing care to the renal inpatient and outpatient

You will be responsible for leading a specialised service, providing nurisng input as a

clinical expert to support and facilitate high quality, individualised care.

You will be a resource for patients and the clinical team, acting a professional role model

providing direct and indirect care.


Main duties of the job

* Provision of specialist, high quality, expert nursing care and support for renal patients(and their carers) with diabetes in liaison with the multidisciplinary team(s) and otherhealth agencies.
* To act as a resource for patients and the clinical team by the provision of
* comprehensive advice, information, teaching and training with regards to diabetes in kidney disease both within ICHT and externally.
* To undertake, where appropriate, nursing research with published outcomes, and updating own knowledge to promote excellence in clinical practice, and the setting of quality standards within speciality.
* Contribute in audit processes. Formulate an annual report/clinical audit of own CNS service and outcomes.

Person Specification


Education

* NMC Registration on relevant part of register
* Minimum of first degree and post registration courses (CPD) relevant to clinical speciality
* Evidence of continuing personal and professional development
* Management or leadership qualification or equivalent experience.
* illingness to undertake nonmedical prescribing course if not already in possession of this
* Has attained or is working towards Master's Degree
* Counselling/Communication course/certification


Skills

* Proven skills in patient assessment within the diabetes care setting.
* Able to adapt to clinical and organisational changes. Proven enthusiasm towards teaching/sharing knowledge
* Evidence of applying current research to practice. Able to demonstrate a working knowledge in standard setting and quality management
* Team worker with excellent interpersonal skills.
* Ability to empathise Self-motivated with drive & initiative
* Ability to work unsupervised.
* Manage, plan & prioritise own workload effectively and meet deadlines
* Calm under reasonable pressure.
* Experience in conducting audits
* Basic IT skills
* Proven oral and written communication skills. Demonstrate the ability to compile complex written information.
* Presentation skills in both formal and informal settings
