Urgent Community Response/Virtual Ward Senior Nurse/Paramedic Pract.

Are you looking for an exciting opportunity in a high performing and supportive team? Perhaps you are ready for your next challenge? We have a fantastic opportunity for an experienced Nurse or Paramedic to join the Wokingham Urgent Community Response and Virtual Ward Team for a 12 months fixed term.

The role will provide clinical assessment skills to patients in their own home and in a care home, who require an urgent response to prevent unnecessary admission to hospital. The post holder will work as part of a multi-disciplinary team to provide appropriate patient focussed care.

The role will provide leadership and line management to junior staff within the multidisciplinary team, including nurses and support workers, and clinical supervision and training/education to nurses and support workers, as required.

You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.

The post holder will work 37.5hrs per week, 7 days a week, between the hours of 8am - 8pm, which will include weekends.

We will also consider this as a secondment opportunity.


Main duties of the job

* To practice as an advanced level clinician, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service and Virtual Ward in collaboration with the Community Geriatrician and GP.
* As an experienced practitioner, you will take responsibility for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.
* The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner.
* To provide leadership and line management to the Band 6 clinicians and Band 3 & 4 healthcare/multi-therapy assistants.
* Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC/HCPC, information governance and local / national guidance.
* Work collaboratively with colleagues and develop nursing and care delivery policies, identifying how policy will impact on practice and service delivery.
* Work flexibly in response to service need across our multiple sites.

Job responsibilities

The must haves for you to be considered for this role:

* Registered General Nurse or Paramedic
* Advanced History taking and physical assessment or working towards it
* Non-medical prescriber or working towards it
* Able to work autonomously, with the ability to work under pressure, with excellent organisational, communication and time management skills to prioritise work

Person Specification


Education/Qualifications/Training

* Registered General Nurse or Paramedic or Physician Associate
* Post graduate diploma or equivalent experience
* Advanced History taking and physical assessment
* Non-medical prescriber or working towards it
* Qualification in Teaching and Assessing


Continuous Professional Development

* Evidence of continuing professional development


Previous Experience

* 3 years post registration experience
* Demonstrate a high level of clinical skills.
* Demonstrate an in-depth knowledge and experience of clinical examination, history taking, pharmacology
* Experience of instigating and interpreting investigations and initiating appropriate plans of care for patients.
* Formal Teaching.
* Evidence of leadership skills.
* Multi-Disciplinary Meetings
* Audit/Research


Knowledge, Skills & Abilities

* Ability to communicate clearly with patients, relatives, and MDT team.
* Knowledge of evidence informed and research-based practice.


Additional Requirements

* Car owner with current driving licence and able to travel to other sites (including visiting non trust sites).
* Adapts to different working conditions

£50,008 to £56,908 a yearper annum (incl of HCAS)
