Health & Justice Services are excited to recruit for the Healthcare Navigator role, a new position within our services. The successful candidate will have strong initiative, transferable skills, and exceptional knowledge of healthcare delivery within prisons. They will support and guide patients in their early days of custody, aiming to reduce self‑harm risk and improve overall experience.
Responsibilities
* Monthly supervision
* Annual personal development plans/appraisals
* Access to internal and external training opportunities
* Preceptorship Programme participation
* Support and guidance with revalidation
* Offer a visible presence during early days in custody and on the induction wing
* Help improve identification of risk factors during early days in custody
* Act as a bridge between HMPPS and healthcare providers to enable joint care improvements
* Address early‑custody vulnerabilities, including reducing self‑harm
* Improve induction into establishments
* Create greater awareness of healthcare and social‑care options and available services
* Support access to services via PODS and the application process, especially for people from diverse backgrounds or with specific needs
* Improve partnership working with services, particularly social care and Safer Custody teams
* Improve engagement rates in healthcare and reduce missed appointments
* Assist with patient choice regarding healthcare interventions
* Further support social prescribing and manage Health Champions
* Run patient forums in conjunction with EPIC, particularly relating to post‑ACCT
* Input into complex case meetings
* Log and make referrals
* Offer appropriate support and guidance to patients and their families/carers
* Plan and organise work using own initiative and teamwork
* Maintain excellent IT skills, including Microsoft Office, Outlook and Excel
* Undertake general office duties to support the role
* Coordinate and participate in internal and external working groups and provide project advice, expertise, and support where requested
