Medicines Optimisation Pharmacy Technician Higher Level

An exciting opportunity has arisen to join our friendly team as a Medicines Management Pharmacy Technician in the Pharmacy and Medicines Optimisation team.
The post is for 32 hours a week over 5 days.
This post will require the successful candidate to work across both Mental Health and Physical Health Hospital Sites.

Job Purpose: To work as part of the pharmacy team at MPFT to ensure the safe, effective and economical use of medicines.
* Work with clinical pharmacists, the dispensary manager and other medicines management pharmacy technicians and pharmacy support workers in providing and developing a comprehensive medicines optimisation service to the wards and clinics.
* Lead the provision and coordination of the Trust Pharmacy led Medicines Reconciliation service in designated areas.
* Support the implementation and delivery of current and future digital based innovations in pharmacy practice.
* Assess patients’ own drugs for use on the wards and document the supplies available on patient drug charts.
* Identify charts that have not yet been clinically screened by a pharmacist.
* Participate in discharge planning to ensure the timely dispensing of discharge medication and final check of patients’ own drugs before discharge.
Person Specification
Qualifications
* BTEC and NVQ Level 3 (or equivalent) in Pharmaceutical Science
* Nationally recognised Accredited Checking Technician Certificate
Experience
* Working as a registered Pharmacy Technician in the UK
* Hospital pharmacy experience
Skills, knowledge and ability
* Able to work as part of a team
* Familiar with Ascribe, Rio, CPMS, SCR and EPMA
