Overview
We are redesigning our community services model and implementing a new Intermediate Care Team. An Occupational Therapist is being appointed on a 0.6 WTE basis to deliver an urgent community response service and play a crucial role in developing the service, providing essential support and care to individuals, with a focus on promoting independence, dignity, and well-being. The objective is to optimise safe and timely discharge from an inpatient stay for patients who need some form of continuing care or to prevent admission with interventions that can be managed at home.
To provide a high standard of Occupational Therapy intervention within the Intermediate Care Service, working across the Cwm Taf Morgannwg University Health Board (CTMUHB) localities, health board sites, various community settings and patient homes.
To contribute to the delivery of training programmes and, with appropriate training, lead training sessions with delegated responsibility from the Senior Occupational Therapist/AHP Lead. To autonomously work alongside a multidisciplinary team ensuring best current, evidence-based clinical management of patients referred within the defined specialist clinical area. The post holder will be an autonomous practitioner with relevant post-registration experience and will undertake specialist clinical duties as required within the defined area. To take delegated responsibility from the Senior Occupational Therapist/AHP Lead within the Intermediate Care Service, managing patients at risk of unnecessary hospital discharge delay or admission and be responsible for providing Occupational Therapy assessment and treatment plans for patients within the service.
Responsibilities
* Deliver high-quality Occupational Therapy interventions within the Intermediate Care Service across localities, sites, and patient homes.
* Autonomously manage patients as part of the multidisciplinary team, applying current evidence-based clinical practices.
* Lead and contribute to training sessions with delegated responsibility from the Senior Occupational Therapist/AHP Lead.
* Develop assessment and treatment plans for patients at risk of discharge delay or admission.
* Collaborate to optimise safe and timely discharge from inpatient settings and reduce unnecessary admissions.
Qualifications and Skills
* Post-registration Occupational Therapist with relevant clinical experience in the defined area.
* Ability to work autonomously and as part of a multidisciplinary team; capable of undertaking specialist clinical duties as required.
* Welsh Skills Desirable: Speaking & Listening at Level 3 (equivalent to CEFR B2) or above. This is advantageous but not essential.
