Cancer Care Coordinator
Support the implementation of the Personalised Care Agenda for people affected by or living with cancer, working closely with cancer nurse specialists and a multidisciplinary team.
Responsibilities
* Assist in providing Holistic Needs Assessments and producing associated care plans.
* Support the implementation and administration of Treatment Summaries.
* Organise health and well‐being events and activities.
* Provide a single point of access, including rapid re‑entry into the system for those identified as having urgent or specialist needs.
* Communicate effectively (daily face‑to‑face, telephone, and email) with patients, relatives, carers, and health/social care professionals.
* Provide general information and support about cancer and cancer services to enable navigation of health and social care and help patients make choices that are best for them.
* Promote equity for patients and ensure they are supported regardless of background.
* Triage telephone and email enquiries using a risk assessment framework, initiating appropriate responses and referring complex decisions to the team.
* Complete all administrative tasks related to electronic Holistic Needs Assessments and care planning.
* Carry out follow‑up work generated by holistic needs care planning, including onward referrals.
* Organise and prioritise workload based on identified needs.
* Coordinate handover with other teams to facilitate safe and effective transition of care between services.
* Plan and deliver Health and Well‑being events.
* Signpost appropriate information and support, encouraging self‑management where appropriate.
* Guide people through the use of self‑assessment resources.
* Evaluate outcomes of care delivery in partnership with the registered practitioner.
* Ensure patients/carers are aware of how to contact the service when conditions or needs change, including out of hours.
* Act as an advocate and facilitator to resolve barriers to care.
