Job Overview

An exciting opportunity to be an integral part of a pilot programme where you will develop the service to provide intermediary support for patients experience of sight threatening conditions.

Working across the multi professional team and integrated patient support services you will develop a patient centred approach to supporting patients to adapt to living with changes to their vision.

As an NHS Health Coach specialising in ophthalmology, you will play a pivotal role in supporting patients with eye conditions to manage their health, follow treatment plans, and improve overall outcomes. Working closely with a multidisciplinary ophthalmology team, you will empower patients to take control of their eye health and general well-being through education, personalised coaching, and lifestyle interventions utilising the TGROW methodology for empowering patients to achieve their health goals.

This position is vital in enhancing patient-centred care by bridging the gap between clinical treatment and everyday self-management, ensuring individuals have the knowledge, tools, and support to maintain their vision and quality of life.

Main duties of the job

Health Coaching:


* Using a coaching methodology such as TGROW you will work with patients one to one initially and then leading to peer support in facilitated group sessions.
* Provide tailored coaching to help patients understand their conditions and make informed decisions about treatment and lifestyle changes.
* Support patients in setting and achieving health-related goals (e.g., medication adherence, smoking cessation, weight management/ lifestyle and diet choices).
* Use motivational interviewing techniques to overcome barriers to adherence and self-care.

Detailed Job Description And Main Responsibilities

Clinical

Health Coaching:

* Using a coaching methodology such as TGROW you will work with patients one to one leading to peer support in facilitated group sessions.
* Provide tailored coaching to help patients understand their conditions and make informed decisions about treatment and lifestyle changes.
* Support patients in setting and achieving health-related goals (e.g., medication adherence, smoking cessation, weight management/ lifestyle and diet choices).
* Use motivational interviewing techniques to overcome barriers to adherence and self-care.

Group Facilitation:

* A key aspect of this role is identifying and engaging small cohorts of patients to participate in structured group coaching sessions.
* Identifying suitable patient who may face similar challenges, such as medication compliance, lifestyle, or coping with vision loss.
* Planning and delivering group sessions:
* Develop and facilitate engaging, interactive group coaching sessions tailored to the needs of patients with conditions like glaucoma or diabetic retinopathy.
* Use evidence-based techniques to support behaviour change, self-management, and peer learning/ support.
* Use facilitation strategies to ensure inclusivity and accessibility for all participants.

Patient Engagement:

* As part of the integrated patient support services act as a point of contact for patients diagnosed with chronic or acute eye conditions (e.g., glaucoma, macular degeneration, diabetic retinopathy).
* Build trusting relationships with patients to encourage participation in their care plans.
* Develop patient evaluation processes for the service provision
* Encourage service users in providing feedback on the quality of service provided in the clinical areas both on a local and Trust level such as friends and family card/question. Participate and provide leadership in implementing user focused quality improvement initiatives

Education & Resources:

* Deliver educational sessions on maintaining eye health and managing conditions.
* Signpost patients to additional resources or support services, including community organisations, social care, or patient groups, including internal referrals to the ECLO department.

Care Coordination:

* Collaborate with ophthalmologists, optometrists, orthoptists, nurses, and other healthcare professionals to provide seamless patient care.
* Ensure that patients understand their treatment plans and follow-up appointments.
* Monitor patient progress and report to the clinical team as needed.

Health Promotion:

* Advocate for eye health by participating in community outreach programs, workshops, and campaigns.
* Promote preventative measures such as regular eye tests and lifestyle modifications to reduce the risk of vision loss.

Training

* To identify own training needs and attend training necessary for continued professional development
* To promote the benefits of health coaching to healthcare professionals through presentations at Quality Forum, Service meetings and communication engagement.

Organisation and Development

* To participate in the audit process in relation to the service and related research and present findings.
* Gather data for audit purposes
* To work as a member of the department’s patient support service team service
* To participate and contribute to and in regular supervision, performance appraisal meetings with the line manager and attend team and service meetings across any site within the organisation
* To undertake all mandatory training annually
* To be aware of issues regarding the safeguarding policies with respect to vulnerable service users and to refer cases to the safeguarding lead.

Administration

* To maintain detailed records the key data sets of the patients seen, the assistance provided, and any courses of action proposed
* Understand and work with data bases such as Microsoft Office software, including Excel. To keeping up to date with new technology and developments.
* Will practice in a cost effective and cost aware manner and contribute to the directorate Cost Improvement Process

Compliance and Governance

* Maintain clear and legible patient records in accordance with both Trust and professional standards for record keeping.
* Working with the lead counsellor develop process and criteria for the service in line with trusts’ governance framework
* Provide data and reports for evaluating the service to key stakeholders in a timely manner

Working Conditions

* May be exposed to difficult patients who may be verbally abusive or aggressive

Person specification

Qualifications

Essential criteria

* Qualified Health Coaching, OR Registered Nursing, Registered Allied Professional or Accredited/Registered Counsellor OR Eye Clinic Liaison Officer with significant experience
* Education to a minimum of functional level 2 or equivalent (GCSEs English and Maths)

Experience

Essential criteria

* Previous Experience in Ophthalmology or Sight loss Rehabilitation

Knowledge

Essential criteria

* Knowledge and Understanding of Eye Health of Common Eye Conditions and their impact on patients quality of life
