About the Role
We are looking for a Health Care Support Worker (HCSW) to join our dynamic District Nursing Service. This role offers an exciting opportunity to work in a community setting, supporting patients in their own homes with person‑centred care.
Responsibilities
* Provide support to nursing team in delivering holistic person‑centred care to patients and their families/carers.
* Work unsupervised in community setting within defined parameters and scope of practice.
* Perform wound care, skin assessment, basic vital signs, venepuncture, catheter care, and provision of equipment.
* Deliver skilled nursing interventions to those on caseload.
Qualifications and Requirements
* Team player with effective communication and interpersonal skills.
* Flexible working essential, working between three bases to support service needs.
* Experience working within the community desirable but not essential.
* Full current driving licence and access to a car essential.

* Post may require PVG Scheme if assessed as a "regulated role", due to legislative changes from 1 April 2025.
