Job overview

The team works closely with acute and community services, providing a comprehensive assessment of patients to avoid admission. The service operates over seven days, between 8am and 6pm and the successful candidate will be rostered regular days of work within this.

The successful applicant will be able to demonstrate a flexible approach to the assessment of older adults in crisis, acknowledging the broad aspect of medical and social factors influencing frailty. In return, we are able to offer a supportive team with multi-disciplinary working, formal education to enhance your professional development and the opportunity to help shape our service.
