Care Coordinator – Home Care
Western CO — Pay Range: $20-$25 an hour
PeopleCare Health Services is hiring a Care Coordinator to manage client caseloads and coordinate quality home care services across various counties in Western Colorado. This role includes assessments, client follow-up, caregiver coordination, scheduling support, documentation, and communication with families, providers, and partner agencies to ensure care is delivered effectively and compassionately.
What You’ll Do
* Manage an assigned caseload of clients
* Coordinate care with clients, families, caregivers, and community partners
* Complete intake assessments and required client visits
* Support caregiver scheduling and coverage needs
* Help address client or staff concerns promptly and professionally
What We’re Looking For
* Healthcare, case management, or caregiving experience preferred
* Strong communication, organization, and problem-solving skills
* Comfortable working independently, in a fast-paced environment, and as part of a team
Work Setting
This role includes working from home, attending occasional in-office meetings, visiting client homes, and traveling throughout the field. Travel is required up to 50% of the time.
