Overview

To assist residents in all aspects of daily living in accordance with individual care plans, monitoring health status and reporting relevant information to the Manager.


Responsibilities

* To be responsible for assessing, planning, implementing and evaluating care to allocated residents on a monthly basis.
* To allocate and facilitate care throughout the shift deploying staff, monitoring and supervising their work to ensure care is provided to a high standard.
* To assist the Manager to cover shifts for the Home and make the Manager aware of any short falls.
* To oversee the receipt/recording/storage/handling/administration and disposal of medicines in accordance with the Home's policies and procedures.
* To monitor and take responsibility for meal times, ensuring all residents have adequate dietary and fluid intake and record input and output charts for those at risk of poor nutrition/hydration. To liaise with GP if necessary.
* To assist in the promotion of continence.
* To ensure that residents' rooms, clothes and belongings are maintained to a satisfactory condition, each resident has adequate clothing and personal toiletries and that chiropody and hair care is maintained on a regular basis.
* To be responsible for regular Quality Assurance checks with residents, relatives and visiting professionals.
* To ensure that residents have at least an annual dental, optical and hearing test.
* To monitor residents' presentation throughout the shift in respect of hair, clothes, dentures, glasses, hearing aids, walking sticks etc.
* To maintain clear, accurate and concise records as per the Home's Record Keeping and Data Protection Policy and Confidentiality Policy.
* To manage the care of the dying and liaise with GP and District Nurses for pain control management.
* To monitor and liaise with GP and District Nurses in respect of wound care management, if required.
* To support, train and supervise junior and new staff members in all aspects of their work under the guidance of the Manager.
* To participate in staff/residents meetings/reviews.
* To deal with emergencies and crisis as per the Home's policy ensuring the recording of accidents/incidents.
* To monitor and report the treatment of residents with reference to the Safeguarding Adults at Risk from Abuse Policy.
* To identify and respond to the changing needs of residents and formulate a plan of action in order to best meet the needs of the residents.
* To monitor the health, safety and welfare of residents, staff and visitors.
* To monitor moving and handling techniques, safe operation of equipment and compliance with food hygiene, infection control, COSHH practices of staff and fire hazards in the Home.
* To monitor standards of cleanliness and tidiness of the environment and equipment (including cupboards, commodes, bathrooms, and residents' rooms).
* To encourage person-centred care, promoting the dignity, privacy, choice, rights, fulfilment and independence of residents (incorporating psychological, social, cultural and spiritual needs).
* To liaise with the Manager if unable to deal with any queries or complaints.
* To assist in developing policies and procedures for the Home in line with the Social Care Act 2008 (Regulated Activities) Regulations 2010 and have an understanding of legislation relating to the Care Home.
* To promote equality, diversity and human rights for residents and staff ensuring non-discriminatory practice by the team (on the grounds of race, colour, gender, age, religion and physical/mental disabilities).
* To promote a positive image of the Home by maintaining a high professional standard and acting as a role model. To maintain and develop good staff morale and communication.
* To have a good understanding of the needs of people suffering from a dementia and their individual communication needs.
* To have a general understanding of residents' medical conditions.
* To monitor safety of equipment/premises on a day to day basis.
* To monitor residents' mobility on a daily basis and liaise with the GP in accessing mobility aids.
* To monitor and arrange activities which meet the individualised needs of residents.
* To ensure that a thorough hand-over takes place from shift to shift.
* To monitor that staff are working in an environment free from violence, harassment, intimidation and bullying.
* To monitor and observe that staff are implementing training in practice.
* To promote community involvement in the Home.
* To monitor security and safety in the Home and its grounds and that all visitors are identified and recorded coming in/out of the building in the 'visitors book'.
* To ensure that, in the absence of the Manager/Deputy, Statutory Notification Forms, as required by the Social Care Act 2008 (Regulated Activities) Regulations 2010, are completed and sent to the Care Quality Commission and the relevant local authority informed accordingly.
* To ensure that, in the absence of the Manager/Deputy, bodies such as the Health & Safety Executive in relation to RIDDOR and notifiable diseases are notified as required by Law and as required in the Home's policies/procedures.
* To ensure that, in the absence of the Manager/Deputy, all persons/organisations are informed of any incidents/events as required in the Home's policies and procedures.
* To undertake any other duties as required which are conducive/contribute to an effective care provision.
