Job overview We are a specialist Physical Health Team that has recently expanded within Adult Community Mental Health Services in Black Country Healthcare NHS Foundation Trust The teams focus is to improve the physical health outcomes for our patients with direction and standards issued by the Care Quality Commission, NHS England and NICE guidelines. You will undertake cardio-metabolic monitoring in Out-patient Clinics and complete some home visits when needed. Advert The role of the STR Worker is to support in Out-patient Clinics and complete some home visits to undertake annual cardio-metabolic physical health checks. You will monitor physical observations; BP, weight/height/BMI, complete ECG’s and take bloods (all training will be provided). You will also support senior clinicians within the one-stop Clozapine Clinic. We are seeking an enthusiastic and motivated individual with experience of working with adults who have severe and enduring mental health difficulties. Applicants must have excellent communication skills, good IT skills and a genuine interest in improving the physical health for patients. You should be able to form good working relationships with the multi-disciplinary team to deliver a safe, high quality and effective service. You will be supported with supervision from senior members of the team. Detailed job description and main responsibilities To provide Support and Time to an allocated group of service users in order to promote their Recovery and physical health monitoring to maintain them in their community environment helping them to build their own support network. To support the individual service user to identify their needs and have these at the fore at all times, working to the Recovery Model to support service users to work towards their identified goals. To support the individual service user with their physical health needs To support the on-going development of the physical health clinics within community mental health services. To support the clinical team in the delivery of high quality health care to the patients. To support the running of Clozapine Clinics.
