We have an opportunity for 1 x part‑time Care Coordinator, based at Royal Primary Care East sites in Chesterfield, working 22.5 hours per week.

Job Responsibilities
* Manage registrations and prescription management for a busy respite care home.
* Provide professional and holistic administration support for all patients discharged to the home, ensuring a smooth and safe transition from hospital to respite to home.
* Support the Administration team with a focus on achieving excellent patient care via telephone and face‑to‑face.
Essential Experience
* Experience working in a pharmacy, healthcare or general practice setting.
* Experience managing clinical administrative tasks / workload.
* Experience working within a multi‑professional team.
Desirable Experience
* Experience registering new patients.
* Experience managing patient prescriptions.
Essential Skills and Knowledge
* Ability to work under pressure and prioritise own workload.
* Ability to manage multiple tasks in a fast‑paced environment.
* Ability to work independently or as part of a team.
* Good interpersonal and customer service skills.
* Good written presentation skills.
* Effective communication with a wide range of stakeholders, creating good working relationships.
Desirable Skills
* Knowledge of clinical systems (e.g., SystmOne).
* Knowledge of NHS prescription and medication guidelines.
* Understanding of Electronic Prescribing Service (EPS).
Qualifications and Training
* GCSE in Maths and English or equivalent qualification.
Desirable Qualifications
* GP Assistant Certificate qualification.
* NVQ Level 2 (or equivalent) in Pharmacy Services or related healthcare qualification, or willingness to work towards it.
