Job summary

Are you looking for an innovative role in a multidisciplinary team? This fantastic opportunity awaits a dynamic, motivated Occupational Therapist looking to develop clinical, operational and leadership skills.

This team also provides a rapid response service for patients aimed at avoiding hospital admission if safe to do so. The Discharge to Assess element aims to support patients come home for assessments and stay at home if safe to or return to hospital to develop a safe discharge plan. The aim is to work closely with other teams to prevent hospital admission, facilitate discharge & promote independence.

As a clinical lead you will be expected to provide clinical leadership and operational management to the team and manage the clinical caseload of patients to ensure safe patient flow.

Main duties of the job

* Providing clinical input, demonstrating advanced competence in complex decision making, assessment & management of clinical needs of a complex caseload.
* Supporting the development of individuals & the team through appraisal, personal development planning, coaching & mentoring.
* Provide assurance & governance for the service.
* Lead the implementation of clinical protocols/pathways in the service.
* Work with the Professional Lead to implement the AHP strategic plan.
* Develop rehabilitation & recovery focused services to enabling people to be independent for longer, manage their health needs & lead healthy, active lives in their community.
* Supporting the service implement clinical effectiveness programs by taking part in any agreed research/audit projects, leading key aspects as appropriate
* Provide strategic clinical leadership using the most up to date evidence based theory.
* Work alongside the Physiotherapy Team Lead to deliver the aims of the ICC & leadership to other community rehabilitation team staff.
* Offer expert clinical advice for service users, carers, staff & other professionals.
* Make a contribution to training, teaching & supervision of colleagues.
* Work in partnership with service users, carers & other agencies to ensure effectiveness of outcomes.

The post holder will have the autonomy, legitimacy and authority to make changes within the clinical practices of the team to move the service forward.
