What the Job Involves The role involves working within All-Age Disability Area Teams to provide a seamless service that addresses the social, emotional, and environmental needs of children and young people with disabilities. By working in partnership with SEND services and other relevant agencies, you will ensure the alignment of social care and EHC plans to promote and safeguard the welfare of vulnerable individuals. Responsibilities include assessing risks and needs, coordinating services, maintaining accurate records, fulfilling statutory requirements, and participating in supervision and professional development.
