Job Description

To represent the Community Care Team in the provision of comprehensive front line information, action and advice service in a professional and courteous manner.

Requirements

To work in partnership to promote information and access to community services and resources that are available for everyone.


To listen and advise people and to identify how they want to live their life, the difficulties they are having and what would make a positive difference.


Liaise with and advise Community Care Staff in regards to clients, verifying and gathering information for assessment or reviews.


Accurate recording of care services entered on Carefirst and ELMS IT systems.


Use of various IT systems.


Participate in team meetings, attend supervision sessions and participate in ERDP.


Deal with challenging relationships and environment.


Main source of Tier 1 provision of community care to members of the public.


Using judgement and discretion daily regarding the level of response to any situation.


Health & Safety.

Responsibilities

To work in partnership to promote information and access to community services and resources that are available for everyone.


As necessary liaise with and work in partnership with appropriate statutory and voluntary agencies working in the field of disability and health in a confidential and sympathetic manner.


Liaise with GP Practices and other agencies to ensure all relevant information is gathered to give comprehensive account when referring on for Tier 2 services.


Liaise and co-ordinate with Police Scotland regarding Appropriate Adult requests and Police Concern reports, maintaining rotas to provide cover for Moray area.


Maintain spreadsheet to provide statistical information.


Liaise with Moray Council training team to provide adequate Appropriate Adult training.


To listen and advise people and to identify how they want to live their lives, the difficulties they are having and what would make a positive difference.


Provide specialist help and advice without the need to refer the enquiry elsewhere, where possible.


Provide first point of contact for hospitals outwith Moray to ensure safe discharges, with regards to Occupational Therapy equipment.


Have an understanding of Occupational Therapy equipment and to advise and order equipment with the support of the Duty Occupational Therapist. Arrange urgent repairs of hoists and stairlifts, co-ordinating other resources if required if repair is delayed. Promote the Independent Living Centre.


Attend to clients at the front desk in the absence of a Social Worker or Occupational Therapist.


Liaise with and advise Community Care staff in regards to clients, verifying and gathering information for assessment or reviews.


Update and advise Duty Officer/OT if the referral is critical.


Accurate recording of all support and services provided entered on Carefirst and ELMS IT systems.


Accurate recording of all enquiries on Carefirst and ELMS.


Accurately record Dementia referrals for statistical purposes, send out appropriate paperwork to clients and refer on to Alzheimer’s Scotland.


Use of various IT systems


Use of various IT systems for example, Word, Excel.


Participate in team meetings, attend supervision sessions and participate in ERDP.


Attend and participate in regular team meetings.


Attend employee training as requested.


Participate in ERDP (Employee Review and Development Programme).


Dealing with Challenging Relationships and Environment


Deal confidently with people who present with challenging behaviour, are abusive and have extreme disabilities and difficulties or those who present with inappropriate behaviour.


Communicate with people who have hearing and sight impairments and those who have difficulty communicating.


Main source of Tier 1 provision of community care to members of the public.


Develop and maintain the resources necessary to provide in depth knowledge of local resources.


Using judgement and discretion daily regarding the level of response to any situation.


Constantly prioritise and keep up to date with all e-mails to the Access Care Team, keeping constant watch on any urgent requests and action as required.


Health & Safety


To take reasonable care for the health and safety of self, other staff and service users as defined by Council policy and other relevant documentation and training.


To co-operate with the Council in ensuring that health and safety duties are complied with.

Closing Date: 1 May 2026


Starting Salary: £27,671.80


Hourly Rate: £14.68


Hours per week: 36.25
