Band 6 Hospital Liaison Practitioner within the Adult Mental Health Hospital Liaison Team.
Location: Woodlands Unit, Rotherham Foundation Hospital Trust Site.
Posting date: 03 June 2026 – Closing date: 03 July 2026.
Responsibilities
* Provide urgent and emergency response to mental health referrals from The Rotherham Foundation Trust (TRFT).
* Conduct full bios‑psychosocial assessments and develop safe, supported discharge plans.
* Collaborate with TRFT staff to enhance their mental health knowledge, deliver training and promote patient‑centred care.
* Work within a flexible 24/7 duty rota as part of a multidisciplinary team.
Qualifications and Experience
* Minimum of one year post‑qualification experience in mental health services.
* Excellent assessment and risk‑assessment skills and the ability to work well under pressure.
* Capacity to work autonomously and as a member of a wider multidisciplinary team.
* Experience working with adults with complex mental health needs.
* Willingness to undertake continued professional development.
