Job Summary
The Care Coordinator plays a vital role in ensuring that clients receive the highest quality of care and support. This position involves coordinating services, managing schedules, and liaising with healthcare professionals and front line care workers to facilitate effective communication and service delivery. The ideal candidate will possess strong organisational skills and a passion for helping others.
Responsibilities
* Coordinate care services for clients by assessing their needs and preferences.
* Schedule appointments and manage calendars for healthcare providers and clients.
* Maintain accurate records of client interactions, care plans, and service updates.
* Liaise with healthcare professionals, family members, and other stakeholders to ensure seamless communication and coordination.
* Monitor client progress and adjust care plans as necessary to meet changing needs.
* Provide administrative support within the office, including answering phone calls and responding to inquiries.
* Assist in the development of care policies and procedures to enhance service delivery.
* Able to use the care management system to ensure that task are completed by carers
Experience
* Previous office experience is essential, demonstrating strong organisational skills and attention to detail.
* Experience in a healthcare setting or familiarity with care coordination is advantageous AND mandatory.
* Proficiency in using office software applications (e.g., Microsoft Office Suite) is required.
* Excellent communication skills, both written and verbal, are necessary for effective collaboration with clients and professionals alike.
