PLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FULL DUTIES OF THE ROLE. The post holder will act in accordance with their active professional body, i.e. NMC, HCPC or SWE. The post holder will support the management of care for people with a learning disability and autism when an inpatient in an acute ward.

This will be both through admission and discharge, working collaboratively with internal and external agencies such as Local Authority and community teams to plan and facilitate safe transitions. The post holder carries continuing responsibility for the holistic assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards for those adults with Learning Disabilities and/or Autism. The post holder will provide specialist clinical care and advice to patients with a learning disability and autism or both, and to the professionals who support them offering advice and guidance on Health Passports and Reasonable Adjustments required to gain best outcomes for the patient. The postholder will contribute to the development of the Learning Disability and Autism Service alongside the learning disability community team by facilitating innovation and the development of evidence based nursing and multi-professional practice.

The post holder will work within the acute Trust to help develop a culture that will support improvement and innovation in clinical practice and empowerment of practitioners. The post holder will be responsible for direct and indirect patient care, in line with Trust policies. The post holder will devise and deliver training to acute colleagues regarding supporting adults with learning disabilities and autism and the role of reasonable adjustments and the liaison nurse. The postholder will engage, enable and empower other professionals across the acute trust services to establish high-quality, compassionate, person-centred healthcare for our patients who have a learning disability and are autistic.
