Health and Wellbeing Practitioner –Haverhill Leisure Centre Salary £25,119.37 FTE- Hourly rate £13.02 Active Living is an award winning Exercise Referral scheme supporting people to increase physical activity and improve health, delivered in partnership with Abbeycroft, West Suffolk NHS Foundation Trust and Allied Health Professionals. Instructors guide patients through 12 or 24 week programmes with follow on options to encourage long term behaviour change. As a Health and Wellbeing Practitioner, you will coordinate the scheme, deliver sessions, and support clients—including those with complex needs—to overcome barriers and improve wellbeing. You will manage referrals, complete assessments, design safe exercise programmes, and develop sustainable activity pathways. Key Responsibilities: • Support daily operations of health pathways, including data collation, client triage, and communication with colleagues, clients, and health partners. • Deliver client triage, health assessments, inductions, and safe, effective exercise prescriptions for the Active Living service. • Maintain clear, professional communication and strong client liaison, handling queries, problem solving, and ensuring high quality service delivery. • Assist with service delivery by covering appointments, classes, and sessions when required, and support ongoing client continuation routes. • Complete client reassessments, evaluations, and accurate data entry to monitor progress and report outcomes. • Identify and support clients with complex needs, addressing barriers, health inequalities, and wider determinants of health. • Uphold organisational values and undertake additional reasonable duties related to the role. Requirements: • Responsible individual with a passion for working in health and well-being. • Strong understanding of wider health determinants and strengths and barriers faced by residents in your locality • Experience of supporting people to access local services to support health and wellbeing • Knowledge of behaviour change theories and techniques and experience of utilising these with clients to make healthy changes • A strong understanding of referral, triage, and assessment processes • Experience of managing a challenging workload through periods of high demand, and adapting to operational pressures • Act as a local ambassador for healthy behaviour change • Proven health coaching skills and ability to effect behaviour change • Strong commitment to safety and the well-being of others. • Ability to work well both independently and as part of a team. Qualifications: • Level 3 Fitness Qualification minimum requirment • Level 3 Exercise Referral Qualification required • Relevant health coaching qualification or an accredited health coaching skills programme preferred • First Aid at work certification Candidates without a Level 3 fitness qualification will not be eligible for this role. First Aid at work certifications can be attained with Abbeycroft.
