Applicant Requirements

Previous experience in a similar role in general practice would be an advantage, but not essential, as training is provided.
Job Responsibilities
* Provide an effective and efficient recall system to patients including chronic disease management such as Diabetes, Asthma etc.
* Filtering data on Microsoft Excel to find appropriate patient groups that need to be recalled for treatment
* Inviting appropriate patient groups to attend their annual reviews via SMS, telephone or other communication methods
* Booking patients in for appointments via our clinical system
* Creating blood forms for monitoring blood tests
* Ensure blood pressures are recorded and actioned correctly
* Having an awareness of the targets and requirements for Long Term Conditions management – for instance, the Quality & Outcomes framework
* Facilitate at least one group review a week alongside the clinician
* Check all incoming patient communication inboxes
* Team working - effective collaboration to ensure completion of shared tasks
* Any other ad hoc tasks required
Working Requirements
* Monday: 8.30 – minute unpaid lunch)
* Tuesday: 8.00 – minute unpaid lunch)
* Wednesday: Day off
* Thursday: 8.30 – minute unpaid lunch)
* Friday: 8.00 – minute unpaid lunch)
Total – 30 hours per week
Working hours need to fit around our Group Consultations days and times.
