Provide a comprehensive clinical pharmacy service to paediatric in-patients and highly specialised paediatric drug therapy advice to patients/carers, medical and nursing staff and other healthcare professionals. Teach pharmacy, nursing and medical staff in specialist areas of clinical practice and supervise less experienced band 6 pharmacists and students.

Job responsibilities
Provide a highly specialist clinical pharmacy service to paediatric inpatients in accordance with professional, national, departmental and Trust policies. Undertake medicines reconciliation, medication review, formulation and review of pharmaceutical care plans and all other aspects of clinical and near-patient pharmacy. Advise on drug interactions, adverse drug reactions, therapeutic drug monitoring and compatibilities of drugs, ensuring a holistic approach to pharmaceutical care when advising on dose changes. Record interventions and clinical advice in patient care plans or medical notes. Produce specialist drug therapy guidelines and contribute to multidisciplinary ward rounds. Participate in specialist clinical patient discussions and guide medical and nursing staff in therapy choices, safe prescribing and administration. Provide and interpret highly specialist pharmaceutical information to consultants, other medical and nursing staff and patients. Liaise with senior medical and nursing staff to plan therapies in advance. Work as a non-medical prescriber in a designated area or work towards this qualification. Provide information for patients/parents and participate in local and national clinical trials and audits. Investigate and report medicines-related incidents or complaints and develop shared-care packages for effective communication across the care interface. Support and develop prescribing and administration of specialist therapies during day-case admissions and at home, ensuring governance and cost-effective drug use. Communicate with consultants on prescription anomalies, monitor toxicity, and participate in evaluating new drugs and treatments. Complete new drug submissions with required evidence and take responsibility for the operation and development of specialist pharmacy services. Deputise for senior pharmacists and provide training to pharmacy, nursing and medical staff. Educate parents and patients on safe medication use and provide information in distressing circumstances in PICU/HDU/Oncology when required. Participate in Continuing Professional Development and rota systems including weekends, late clinics and on-call pathways.
Person Specification
Education and Training
* Essential: One year pre-registration training; 4 year Master’s degree in pharmacy; Registration with General Pharmaceutical Council; Member of Royal Pharmaceutical Society
* Desirable: Clinical diploma or higher degree; Accredited tutor for postgraduate diploma; Non-medical prescribing qualification
Experience
* Essential: Three years post-registration experience with highly developed specialist knowledge; Two years hospital pharmacy experience; Experience delivering clinical pharmacy to a broad range of specialties and providing highly specialist advice
Knowledge
* Desirable: Computer and IT skills

* Essential: Ability to analyse complex information and apply judgement; excellent organisational and prioritisation skills; ability to communicate complex drug information with patients/parents; up-to-date CPD portfolio; good presentation skills; word processing skills for prescription information and reports
