Are you passionate about supporting people affected by dementia? Join Dougie Mac’s growing Dementia Services Team and make a meaningful difference to the lives of patients, carers, and families across Stoke-on-Trent and North Staffordshire, working within our hospice and the community.
As a Senior Health Care Support Worker, you’ll work closely with our Admiral Nurse to provide holistic, timely, and compassionate support – offering advice and support whilst managing your own caseload. You’ll help reduce crisis points, improve wellbeing, and empower families to live well with dementia.
🔑 What You’ll Be Doing
• Provide personalised, non-complex support and advice to people with dementia and their carers
• Manage your own caseload and conduct community visits
• Support Admiral Nurses with joint visits for more complex cases
• Build trusting relationships with families and recognise early signs of carer strain
• Maintain accurate clinical records using electronic systems
• Support care planning and enablement-focused goals
• Participate in MDT meetings and contribute to service development
👤 What We’re Looking For
✔ Level 3 qualification in Health & Social Care (or equivalent) is essential
✔ Experience supporting individuals affected by dementia, ideally in a community or palliative care setting
✔ Excellent communication and interpersonal skills
✔ Ability to work autonomously, prioritise effectively, and manage your own caseload
✔ Comfortable using IT systems and patient databases
✔ A person-centred approach, aligned with Dougie Mac’s CARE values
✔ Full UK driving licence and access to a vehicle for community visits

Hours: 37.5 hours
