Main duties of the job
1. Identify risks through use of appropriate assessments, act immediately if the situation requires, and ensure all risks are reported to senior clinicians/line managers.
2. Implement and support detailed care plans, outcome measures documentation, risk assessments and treatment programmes (including intervention Support Plans, mental health and multi-disciplinary care plans)
3. Review the 'Duty Inbox' for the team and signpost service users accordingly
4. Offer interventions in line with 'Overcoming Challenging Times Plan'
5. You will work without direct supervision to undertake assessments and deliver interventions relating to individual needs.
6. Working collaboratively with other Trust services to provide seamless care for service users in the Integrated Mental Health Teams

The role of the rapid responder is to provide support in bridging the gap between the service user and their next clinical contact, addressing any unplanned service user requests.

Qualifications
Essential
21. Foundation degree in relevant subject or Diploma/NVQ 3 in Health and Social Care or accredited prior learning with competency based evidence supplemented by short courses.
Experience

22. Significant experience of working with adults / older adults with Mental Health Needs
23. Experience of working within a multi-agency framework

24. Previous work with people with complex and high risk presentations
